Edit and delete cause of death reports

The cause of death reasoning can only be added, never edited or deleted. For example, if there are duplicate entries for the same thing such as an entry saying "runt" and another entry also saying "Runt" but with a capital letter. Giving the ability to edit the cause of death entries will stop this from happening and stop cluttering the cause of death report field.
